A sample of about 200 hair strands measuring 1.5 inches close to the scalp, equivalent to a #2 pencil's thickness, yields around 100mg, which is optimal for screening and confirmation. For EtG, additional analyses, and tests beyond 10 panels, 150mg of the sample is advised. We recommend using a jeweler's scale for weighing the specimen. If scalp hair isn't available, a comparable amount of body hair can be collected. When talking about head hair, it specifically means scalp hair. 'Body hair' defines all other hair forms (such as facial, underarm, etc.).

Process Overview

There are four primary phases in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab's system. This phase verifies if the sample was sealed and transported accurately, assigns a random Laboratory Accessioning Number (LAN), and finishes any supplementary data input not given by an electronic custody chain system.

Screening is a rapid preliminary evaluation for drug misuse. Though Screening is an efficient method to exclude drug usage in most samples, for legal admissibility, a positive screen must be verified. Samples presumed positive in Screening must undergo a secondary confirmation.

For samples assumed positive in the Screening phase, additional hair is extracted from the original sample and made ready for Extraction. In this phase, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or saliva), which makes hair drug screening the most intricate method to conduct.

Any positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples assumed positive receive washing before confirmation as required. The full laboratory process, from Accessioning through Confirmation, is assessed under the CAP (College of American Pathologists) Hair qualification and the accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
