Process Overview

The lab processing of a drug test result encompasses four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses initial sample processing within a lab's system. This phase confirms proper sealing and shipping of the sample, assigns a LAN (Laboratory Accessioning Number), and addresses any extra data entry absent from electronic chain of custody procedures.

Screening serves as a preliminary check for substances. While it's a cost-efficient method to discount drug use in the majority of samples, confirmed positive results are necessary for legal validity. Presumptively positive samples during Screening necessitate further confirmation.

For samples deemed presumptively positive during Screening, more hair is sourced from the original sample and readied for Extraction. At this stage, drugs are isolated from hair at notably lower concentrations compared to other methods (e.g., urine or oral fluid), rendering hair drug screening a particularly challenging methodology.

Confirmation for any preliminary positive results is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, presumptive positive samples are cleansed when required. The entire pathway from Accessioning to Confirmation is evaluated following both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
