Yet, for detecting recent 5-day drug use, urine tests remain the most precise.

What it is, How it works

Hair assessments are now deemed an effective method to uncover substance and alcohol consumption. Hair locks in biomarkers along its shaft, giving a comprehensive history of alcohol and drugs. When collected near the scalp, it allows for a detection period of approximately three months for these substances. Hair collection is straightforward, relatively tamper-proof, and convenient for transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp offers 100mg of hair, which is optimal for analysis. For EtG, add-ons, or tests exceeding 10 panels, 150mg is suggested. Using a jeweler's scale for measuring the specimen is advisable. In the absence of scalp hair, an equivalent amount of body hair can be used. Our reference to head hair pertains solely to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The process involves four primary steps in testing for drugs in a lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into the laboratory's system. This step verifies the correct sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and includes any extra data entry not captured by an electronic chain of custody.

Screening is a preliminary rapid assessment for substance abuse. While screening is a cost-effective strategy to dismiss drug use in most samples, positive findings must be verified to hold legal credibility. Any samples that test presumptively positive in Screening require subsequent confirmation.

If a sample tests presumptively positive at the Screening phase, additional hair is extracted from the primary specimen for further processing in Extraction. Here, drug components are isolated from hair at significantly lower concentrations than in other testing methods (e.g., urine or oral fluids), making hair drug screening a very challenging methodology.

Confirming any positive Screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation when necessary. The entire lab process, from Accessioning to Confirmation, is inspected in line with both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 certifications.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
