To acquire the ideal sample for both screening and confirmation, a sample size of 1.5 inches, approximately consisting of 200 strands of hair (roughly equivalent to the thickness of a #2 pencil), will provide 100mg of hair. For EtG, additional tests, and/or those exceeding 10 panels, a sample of 150mg is advised. We suggest using a jeweler’s scale to measure the specimen. In the absence of enough scalp hair, an equivalent volume of body hair can be utilized. When head hair is mentioned, it specifically refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four integral steps involved in the laboratory evaluation of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into the lab's database. This step confirms the proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and handles any further data entry not covered by an electronic chain of custody system.

Screening includes an initial swift assessment for abuse substances. While it is an economical approach to eliminate drug usage across most samples, a positive screening result must be validated for court admissibility. Samples that are tentatively positive during Screening necessitate additional confirmation.

Should a sample be tentatively positive in the Screening phase, additional hair is taken from the initial sample for the Extraction process. This process involves the drug extraction of hair at significantly reduced concentrations compared to methods like urine or oral fluid tests, asserting the complexity of hair drug screening.

The Confirmation of a positive screening outcome utilizes GC/MS, GC/MS/MS, or LC/MS/MS methods. All tentative positive samples undergo cleansing prior to confirmation if necessary. The comprehensive laboratory procedure, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
