For EtG testing, extended panels, and/or tests beyond 10 panels, 150mg of specimen is suggested. Utilizing a jeweler’s scale for weighing the specimen is advisable. If scalp hair is not accessible, an equivalent quantity of body hair can be collected. Here, head hair indicates scalp hair alone, whereas body hair pertains to all other varieties (like facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four pivotal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a specimen within a lab's system. The procedure verifies that the sample has been securely sealed and appropriately transported, assigns a distinct LAN (Laboratory Accessioning Number), and undertakes any leftover data entry not included in an electronic chain of custody system.

Screening signifies the initial swift assessment for detecting substances of abuse. Although Screening is an economical approach for excluding drug usage in most samples, a confirmed result is required for it to be valid in legal settings. Any samples that return a presumptive positive result during Screening necessitate tertiary confirmation.

For samples preliminarily positive in the Screening phase, additional hair is extracted from the initial batch and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), which contributes to hair drug screening being amongst the most intricate methodologies to execute.

Verification of any positive screening output is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive findings undergo necessary washing prior to confirmation. The comprehensive lab process from Accessioning to Confirmation undergoes review based on both the CAP (College of American Pathologists) Hair designation and conformance to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
