What it is, How it works

A 1.5-inch segment of approximately 200 hair strands (comparable in size to a #2 pencil) near the scalp yields 100mg of hair, the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample on a jeweler’s scale. Absent scalp hair, an equal portion of body hair can be taken. When indicating head hair, we mean scalp hair exclusively. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The core steps within the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample’s initial processing into a laboratory’s database. This encompasses ensuring the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening entails a quick initial examination for drugs of abuse. While Screening is a cost-efficient means to negate drug usage in most samples, a positive screen needs confirmation to be compliant in court. Any samples provisionally positive in Screening do require a secondary confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this stage, drugs are withdrawn from hair at a notably lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All provisional positive samples are cleansed prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
