What it is, How it works

When taken close to the scalp, it offers a detection window of around 3 months for alcohol and drugs. Hair collection is straightforward, somewhat hard to tamper with, and easy to transport.

A sample of 1.5 inches consisting of approximately 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg, which is the optimal amount for analysis. For EtG, additional screenings, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. Measuring the specimen with a jeweler's scale is suggested. In absence of scalp hair, an equivalent amount of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair includes other types such as facial, axillary, etc.

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where the sample is processed into the laboratory's system. This step ensures the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry if not provided by an electronic chain of custody system.

Screening provides a preliminary check for substance abuse. While it is cost-efficient for eliminating drug usage in most samples, a positive screen requires confirmation for court purposes. Any samples that are presumptively positive in this stage need confirmation.

Should a sample show a presumptive positive in Screening, additional hair is extracted from the primary specimen for the Extraction phase. Here, drugs are separated from hair at lower concentrations compared to other methods (e.g., urine, oral fluid), which explains why hair drug screening is technically challenging.

Any positive screening outcome undergoes Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is reviewed under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
