A sample measuring 1.5 inches with approximately 200 strands (around the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred amount for testing and confirming. For EtG, supplemental tests, and/or panels exceeding 10, a 150mg specimen is advised. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a lab’s database. This step includes confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not covered by an electronic chain of custody system.

Screening includes an initial cursory examination for illegal substances. Although Screening is an economical approach to exclude drug use for the majority of samples, a positive screening requires confirmation to be valid in legal proceedings. Any samples with a presumptive positive result in Screening must undergo a confirmatory second test.

If a specimen shows a presumptive positive in the Screening phase, additional hair is extracted from the original collection and conditioned for Extraction. During this phase, substances are withdrawn from hair at notably lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples identified as presumptively positive are cleansed prior to confirmation if necessary. The comprehensive process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
