What it is, How it works

Hair captures a long-term record of alcohol and drug use by embedding biomarkers within the fibers of the hair as it grows. When taken near the scalp, hair may present up to a three-month detection period for alcohol and drugs. Hair collection is straightforward, relatively resistant to tampering, and convenient for shipping.

A 1.5-inch hair sample comprising about 200 strands (comparable to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for initial screening and confirmation. For EtG, add-ons, and/or analysis exceeding 10 panels, a 150mg sample is suggested. It’s advisable to use a jeweler's scale for precise specimen measurement. If scalp hair can't be used, an equivalent quantity of body hair is an alternative. Head hair refers exclusively to scalp hair. Body hair refers to other types, such as facial and axillary hair.

Process Overview

The four key phases in the lab processing of a drug detection result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase of entering a sample into the laboratory system. This step includes confirming that the sample was securely sealed and dispatched correctly, assigning a random Laboratory Accessioning Number (LAN), and finalizing any remaining data entry not supplied by an electronic chain of custody system.

Screening is an initial assessment to check for the presence of drugs. Although Screening is a cost-effective approach to exclude drug use in most samples, a positive result mandates confirmation for legal admissibility. Presumptive positives in Screening necessitate a secondary confirmation.

Should a sample be presumed positive during Screening, additional hair is retrieved from the initial specimen for Extraction preparation. Here, drug compounds are isolated from hair at markedly lower concentrations than in other techniques, such as urine or oral fluid, making hair drug screenings particularly rigorous.

Confirmation for any initial positives is achieved using methods like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo a cleansing before confirmation as needed. The whole laboratory process, from Accessioning to Confirmation, is scrutinized per both CAP (College of American Pathologists) Hair standards and the ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
