Hair testing is increasingly recognized as an effective method for detecting the use of drugs and alcohol. Hair captures long-term drug and alcohol use by embedding biomarkers within the strands of growing hair. When collected near the scalp, hair can offer a detection period of up to roughly three months for both alcohol and drugs. This method is straightforward to gather, relatively challenging to tamper with, and convenient to transport.
A 1.5-inch sample, consisting of around 200 hair strands (similar in size to a #2 pencil) closest to the scalp, provides 100mg of hair, which is the ideal sample volume for analysis and confirmation. For EtG testing, additional screenings, and/or tests involving more than ten panels, a 150mg sample is advised. Measuring the specimen with a jeweler's scale is recommended. Should scalp hair be unavailable, a comparable amount of body hair can be collected. References to head hair pertain to scalp hair exclusively, whereas body hair encompasses other types (such as facial and axillary).
Process Overview
The laboratory examination of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves initially integrating a sample into a laboratory system. This step ensures proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.
Screening provides an initial quick assessment for drugs of abuse. Although screening is a cost-effective method to eliminate drug use in most samples, a positive screen necessitates confirmation for court admissibility. Presumptively positive samples during screening require a subsequent confirmation.
When a sample is deemed presumptively positive during screening, additional hair is extracted from the original specimen and readied for the Extraction phase. This phase extracts drugs from hair at concentrations significantly lower than other methods (such as urine or oral fluid), thus making hair drug screening a challenging technique.
Any positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that exhibit presumptive positivity are washed prior to confirmation if necessary. The complete lab process, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Note: Although commonly termed as “hair follicle tests,†the analysis examines the hair strand rather than the follicle beneath the scalp.
