For tests involving EtG, additional analysis, or those exceeding 10 panels, a 150mg sample is suggested. We recommend weighing the specimen using a jeweler’s scale. If scalp hair is unobtainable, an equivalent amount of body hair can be used. When discussing head hair, this specifically refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test result in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially entering a sample into the laboratory's system. This step includes confirming that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening provides an initial, rapid assessment for drugs of abuse. It serves as a cost-effective method to eliminate drug use in most samples, but a positive screen requires confirmation to be valid in legal proceedings. Any tentative positives in Screening require further confirmation.

If a sample appears positive in Screening, additional hair is extracted from the initial sample for Extraction. This phase isolates drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), thus making hair drug testing the most challenging method to execute.

Confirmation of any positive results from screening is performed using GC/MS, GC/MS/MS or LC/MS/MS techniques. All tentatively positive samples are cleansed prior to confirmation as necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
