A 1.5-inch lock of approximately 200 hair strands (the size of a #2 pencil) nearest the scalp yields 100mg, which is the optimal amount for both screening and confirmation. For EtG, extra tests, and/or panels exceeding 10, 150mg of sample is advised. It's suggested to weigh the hair using a jeweler's scale. If scalp hair is absent, the same quantity of body hair can be used. When mentioning head hair, it refers strictly to scalp hair. Other hair types (facial, axillary, etc.) are classified as body hair.

Process Overview

The primary stages in laboratory drug test result processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with incorporating a sample into the lab's system. This step includes checking if the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any unprovided data entry not handled by the electronic chain of custody system.

Screening is a preliminary assessment for substance abuse. Screening serves as an economical approach to eliminate drug presence in most samples, but any positive screening must be validated to hold up in court. Samples marked as presumptively positive during Screening must undergo further confirmation.

If deemed presumptively positive in the Screening phase, additional hair is drawn from the original sample and readied for Extraction. In Extraction, substances are extracted from hair at substantially lower concentrations than other methods (e.g., urine, oral fluid), which makes hair drug screening particularly challenging.

GC/MS, GC/MS/MS, or LC/MS/MS are employed to validate any positive screening outcomes. Before confirmation, presumptive positive samples are washed as necessary. The entire laboratory methodology from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
