For tests like EtG, add-ons, or tests exceeding 10 panels, 150mg of specimen is suggested. We advise utilizing a jeweler's scale for specimen measurement. Should scalp hair be unattainable, an equivalent volume of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair, on the other hand, includes all other hair variants (facial, underarm, etc.).

Process Overview

The drug test result processing in a lab encompasses four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Samples showing presumptive positivity in Screening necessitate further confirmation.

A presumptive positive outcome in the Screening phase leads to more hair being extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at a significantly lower concentration compared to other methods (like urine or oral fluids), highlighting the complexity involved in hair drug screening.

A positive result from the initial screening is validated using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples showing presumptive positivity undergo a washing process prior to confirmation as required. The complete laboratory protocol, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair qualification and accreditation in line with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
