For EtG, extra tests, and/or panels beyond 10, a 150mg sample is advised. We suggest measuring the sample with a jeweler's scale. Substitute hair from the body can be used if scalp hair is not available. When specifying head hair, it refers exclusively to scalp hair. All other types (facial, axillary, etc.) fall under body hair.

Process Overview

The drug test result processing in the laboratory involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes inputting a sample into a lab's system initially. This step checks if the sample was properly sealed and sent, assigns a unique LAN (Laboratory Accessioning Number), and enters any further data not handled by an electronic chain of custody.

Screening provides a preliminary rapid assessment for substance abuse. Although it’s a cost-efficient method to eliminate drug use for most samples, a positive screening needs validation to be considered in legal settings. Presumptive positive samples in Screening require further confirmation.

If a Screening sample shows a presumptive positive, additional hair is extracted from the original sample for the Extraction phase. Here, substances are removed from the hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluids), highlighting the complexity of performing hair drug screening.

Positive screenings are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing before confirmation, if necessary. The complete process, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair designation alongside ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
