What it is, How it works

Hair analysis is now recognized as an effective means for identifying drug and alcohol consumption. Hair retains a prolonged history of substance use by encapsulating biomarkers within the hair strand fibers. Obtained near the scalp, it can offer up to a 3-month detection timeline for substances. Hair is straightforward to gather, relatively resistant to tampering, and simple to courier.

A 1.5-inch segment comprising about 200 hair fibers (equivalent to a #2 pencil size) closest to the scalp will yield 100mg of hair, the prime sample for testing. For EtG, enhancements, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. We advise measuring the specimen using a jeweler’s scale. If scalp hair is not present, an equivalent amount of body hair may be gathered. Referring to head hair specifically infers scalp hair only. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial sample processing into a laboratory’s framework. This step includes checking that a sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any further data entry absent in an electronic chain of custody system.

Screening entails a preliminary rapid inspection for substance misuse. Though Screening is a cost-efficient method to exclude drug usage in most samples, a positive screen necessitates confirmation for legal admissibility. Presumptively positive samples in Screening do call for an additional confirmation.

If a sample indicates a presumptively positive during Screening, additional hair is collected from the initial sample for Extraction. This process extracts drugs from hair at lower concentrations than other methods (e.g., urine or oral fluids), which is why hair analysis is considered the most challenging methodologically.

Verification of any positive screen result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation when required. The entire lab procedure from Accessioning to Confirmation is assessed following the CAP (College of American Pathologists) Hair classification and accreditation to ISO / IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
