Hair analysis is recognized as an effective method for detecting drug and alcohol consumption. Hair records a prolonged history of alcohol and other drug usage by capturing biomarkers within the fibers of the growing hair shaft. When collected close to the scalp, hair can offer up to roughly a 3-month detection period for alcohol and other drugs. Hair is straightforward to gather, somewhat resistant to tampering, and uncomplicated to transport.
A 1.5-inch sample of roughly 200 strands of hair (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimum sample for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is recommended. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be collected. In reference to head hair, we mean scalp hair only. Body hair includes all other hair types (such as facial, axillary, etc.).
Process Overview
The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails the initial preparation of a sample in a laboratory’s system. This involves verifying that the sample was sealed and shipped accurately, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not provided by an electronic chain of custody system.
Screening provides a preliminary rapid evaluation for drugs of abuse. Though Screening is a cost-efficient method to rule out drug usage in a majority of samples, a positive screen must be confirmed to be valid in legal proceedings. Any samples deemed presumptively positive during Screening need a secondary confirmation.
If a sample is deemed presumptively positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at a notably lower concentration than with other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging methodology to execute.
Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
