A sample size of 1.5 inches comprising roughly 200 strands (similar to a #2 pencil) from nearest the scalp garners 100mg of hair, serving as an optimal specimen for testing and verification. For EtG, additional tests, or over 10 panels, a collection of 150mg is advised. We suggest measuring the sample using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be utilized. Head hair specifically refers to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

In laboratory testing for drug results, there are four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the sample’s preliminary integration into the lab’s framework. This step verifies the sample’s sealed and properly shipped status, allocates a random LAN (Laboratory Accessioning Number), and accomplishes any additional necessary data entry not facilitated by an electronic chain of custody.

The Screening process entails a preliminary evaluation for drug presence. Although Screening is a cost-efficient method for excluding drug usage in most cases, a positive screen requires confirmation to be court-admissible. Presumptuous positives in Screening necessitate subsequent verification.

When a sample tests presumptively positive during Screening, more hair is extracted from the original sample for the Extraction phase. Drugs are extracted from hair at notably low concentrations, making it a more challenging method compared to urine or oral fluid tests.

Confirmation of screening positivity is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are washed before confirmation if needed. The entire lab protocol from Accessioning to Confirmation complies with both the CAP (College of American Pathologists) Hair directive and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
