For EtG, additional tests, and examinations exceeding 10 panels, 150mg of the sample is suggested. It is advisable to measure the specimen using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair may be used. The term head hair specifically refers to scalp hair, while body hair encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s database. This stage confirms the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data input not available via an electronic chain of custody protocol.

Screening entails a preliminary rapid assessment for drugs. Screening is a cost-efficient method to negate drug use for most samples, yet a positive screen requires confirmation to hold in a legal setting. Any samples preliminarily positive during Screening must undergo further affirmation.

If a specimen tests presumptively positive during the Screening phase, additional hair from the initial sample undergoes preparation for Extraction. Throughout this phase, drugs are withdrawn from hair at a significantly lower concentration than alternative methods (for example, urine or oral fluid), which renders hair drug screening the most intricate methodology to execute.

The validation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation where necessary. The full laboratory workflow from Accessioning to Confirmation is assessed following both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
