A 1.5-inch section of roughly 200 hair strands (comparable to the diameter of a #2 pencil) taken nearest to the scalp provides 100mg of hair, which is the optimal sample for screening and confirmation tests. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. It is advisable to weigh the sample using a jeweler's scale. If scalp hair cannot be obtained, a similar amount of body hair may be used. When mentioning head hair, we only mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the phase of entering a sample into a lab's system. It ensures the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and finishes any extra data entry not covered by an electronic chain of custody setup.

Screening is a preliminary quick procedure for identifying drugs of abuse. Although Screening is a cost-efficient means to rule out drug usage in most samples, a positive screening needs confirmation to be suitable for court. Samples that are presumptive positives in Screening require a second confirmation.

If a sample is presumptive positive at the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. This stage involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing the most intricate methodology to perform.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
