In recent years, the use of hair follicle testing has become more common in drug testing. Employers, courts, and substance abuse professionals increasingly require hair over urine tests. Hair drug tests are favored due to their longer detection window, up to 90 days, observed collection, and resistance to cheating. Despite this, urine tests remain the most reliable for recent drug use within the past 5 days.

What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol use. Hair strands capture biomarkers within their fibers as they grow, providing a comprehensive history of substance use. When sampled from near the scalp, hair can offer a detection period for drugs and alcohol spanning up to roughly three months. Hair samples are easy to collect, challenging to tamper with, and uncomplicated to transport.

A 1.5-inch sample consisting of around 200 hair strands (comparable to the thickness of a #2 pencil) near the scalp provides 100mg of hair, considered an optimal sample for both screening and confirmation tests. For tests such as EtG or panels beyond 10, it's advisable to collect 150mg of the sample. We suggest using a jeweler’s scale to determine the sample weight. In cases where scalp hair is inaccessible, a similar quantity of body hair may be collected. The term head hair specifically refers to scalp hair, while body hair encompasses all other hair types (like facial and axillary).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is incorporated into the laboratory setup. This process includes ensuring the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any data entry that isn't covered by an electronic chain of custody system.

Screening is an initial, efficient check for the presence of drugs. While Screening is a cost-effective measure for identifying drugs in most samples, a positive screen must be legally confirmed. Samples that test presumptively positive during Screening require a secondary confirmation process.

Should the Screening step yield a presumptive positive, additional hair is used from the original sample for Extraction. During Extraction, drugs are isolated from hair at lower concentrations compared to other testing methods (such as urine or oral fluid), making hair drug screening a complex process.

Positive Screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positive outcomes are washed before confirmation as necessary. The complete lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Long detection span: Hair drug tests detect drug use for up to 90 days, as opposed to urine tests with shorter detection periods.
  • Hard to manipulate: Hair drug tests are difficult to alter, leading to reliable results.
  • Historical insight: They offer a view of drug consumption patterns over time, not just recent usage.

Limitations:

  • No recent use detection: Drugs typically appear in hair within 5-7 days after use.
  • Expense: Hair drug tests are usually more costly than other drug testing methods.
  • Result variability: Factors such as hair color and personal variations in hair growth may impact drug metabolite concentrations in hair.

Note: Despite the common term "hair follicle tests", the analysis is conducted on the hair shaft, not the follicle beneath the scalp.
