For tests like EtG, add-ons, and those with more than 10 panels, a specimen of 150mg is advised. Using a jeweler’s scale for weighing the specimen is recommended. If scalp hair cannot be obtained, a comparable quantity of body hair can be used. When we mention head hair, we specifically refer to scalp hair. Body hair encompasses all other types of hair (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This step verifies that the sample was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not handled by an electronic chain of custody system.

Screening involves a preliminary assessment for substances of abuse. Though Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be verified for court admissibility. Specimens with presumptive positive results during Screening require a follow-up confirmation.

If a sample exhibits a presumptive positive result at the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. During this stage, drugs are extracted at much lower concentrations than in other methods (such as urine or oral fluid testing), which makes hair drug analysis the most challenging method to execute.

Confirmation of any positive Screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed as required prior to confirmation. The comprehensive laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair specification and the ISO / IEC 17025 accreditation standards.
