For EtG, additional tests, and analyses exceeding 10 panels, a 150mg specimen is advised. Utilizing a jeweler’s scale to weigh the specimen is recommended. In cases where scalp hair is not accessible, a comparable quantity of body hair can be utilized. Reference to head hair pertains exclusively to scalp hair, while body hair includes all other hair types (facial, axillary, and others).

Process Overview

The laboratory procedure for processing drug test outcomes comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial integration of a sample into a laboratory’s system. This process confirms that the sample is properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any supplementary data input absent in an electronic chain of custody framework.

Screening occurs as an initial prompt evaluation for drugs of abuse. While Screening serves as a cost-efficient method to eliminate drug use for most samples, any positive results at this stage must be validated for legal purposes. Samples deemed presumptively positive during Screening require a follow-up confirmation.

When a sample indicates a presumptive positive during the Screening phase, additional hair is extracted from the original specimen and processed for Extraction. Here, the concentration of drugs is extracted from hair at much lower levels than other techniques (such as urine or oral fluid), positioning hair drug screening as a particularly challenging method to execute.

Any affirmatively screened results undergo Confirmation through techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens are cleaned before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, is assessed in compliance with both the CAP (College of American Pathologists) Hair qualification and accreditation standards of ISO / IEC 17025.
