What it is, How it works

Hair testing is increasingly recognized as a potent tool for detecting drug and alcohol consumption. Hair offers an extended timeline for drug and alcohol detection by capturing biomarkers within the strands of the growing hair. By collecting hair near the scalp, one can achieve a detection range of up to roughly 3 months for drugs and alcohol. Hair collection is straightforward, not easily tampered with, and convenient to transport.

Securing a 1.5-inch section of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest the scalp results in 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, a 150mg specimen is advised. It is advisable to measure the specimen using a jeweler's scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. When discussing head hair, it exclusively refers to scalp hair, whereas body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with processing a sample into the laboratory's system, which includes verifying the sample’s seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any pending data entry absent from an electronic chain of custody system.

Screening executes an initial rapid test for substances of abuse. Although Screening is a cost-effective method to dismiss drug use in most cases, any affirmative screen must be confirmed for legal admissibility. Samples found presumptively positive during Screening necessitate a secondary confirmation.

Upon a positive presumption in Screening, additional hair is extracted from the initial specimen and prepared for Extraction. During Extraction, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Positive results from Screening undergo Confirmation using GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples are cleaned before Confirmation if required. The entire lab procedure from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation as well as the ISO / IEC 17025 accreditation standards.
