A sample size of 1.5 inches, consisting of roughly 200 hair strands (about the diameter of a #2 pencil) nearest the scalp, yields 100mg of hair, the preferred amount for initial testing and confirmation. For EtG tests, add-ons, or screenings exceeding 10 panels, a specimen amount of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a similar quantity of body hair can be utilized. When we mention head hair, it specifically implies scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first phase, which involves processing the sample into a lab’s system. This step checks that the sample was sealed and sent appropriately, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry not handled by an electronic chain of custody system.

Screening is the next phase, providing an initial rapid assessment for drugs of misuse. Although Screening is an economical method to eliminate drug usage in most samples, a positive result from Screening requires confirmation for court admissibility. Samples showing presumptive positive results in Screening need additional confirmation.

If a sample results in a presumptive positive during Screening, more hair is extracted from the initial sample for the Extraction process. Here, drugs are extracted from hair at lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening presents the highest level of difficulty among testing methodologies.

The Confirmation process for any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, all presumptive positive samples undergo necessary washing. From Accessioning to Confirmation, the entire lab process is conducted under the CAP (College of American Pathologists) Hair designation and follows accreditation to ISO / IEC 17025 standards.
