What it is, How it works

When hair is collected near the scalp, it can offer a detection period of nearly three months for drugs and alcohol. The collection process is straightforward, the chance of adulteration is minimized, and it is easy to transport.

A sample measuring 1.5 inches, consisting of around 200 hair strands (similar to a #2 pencil's size), closest to the scalp yields 100mg of hair, which is the optimal sample quantity for screening and confirmation. For tests involving EtG, additional panels, or those exceeding 10 panels, 150mg of the sample is advised. The sample weight should ideally be measured on a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be used instead. When we refer to head hair, it specifically means scalp hair, whereas body hair includes other types such as facial or axillary hair.

Process Overview

The laboratory processes a drug test result through four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial sample processing into the lab's system, including validating proper sample sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and completing additional data entry if electronic chain of custody data is lacking.

Screening is an initial rapid check for drug presence. Although cost-efficient for ruling out drug use in most samples, screenings require confirmation to be legally upheld. Any samples that preliminarily test positive in Screening necessitate secondary confirmation.

If screening indicates a presumptive positive, more hair is sourced from the initial sample for the Extraction stage. This phase involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluids), which makes hair drug screening notably challenging to execute.

Confirmation of any positive screening findings is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Samples showing presumptive positivity undergo washing prior to confirmation as needed. The entire lab procedure, from Accessioning to Confirmation, complies with both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
