Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage where a sample undergoes initial processing within a laboratory setup. This phase ensures proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and concludes any supplementary data entry that the electronic chain of custody system doesn’t cover.

Screening entails a preliminary scan for drug misuse. Though Screening is cost-efficient for broad negation of drug presence, a positive screen mandates further verification for legal validation. Samples with presumptive positives from Screening proceed to additional confirmation.

When a sample exhibits presumptive positives during the Screening phase, additional hair is extracted from the original sample for the Extraction stage. Here, drugs are isolated from hair at considerably reduced concentrations in comparison to alternative methods (e.g., urine, saliva), which makes hair drug testing the most challenging technique.

Confirmation of any detected positive during screening is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirming, all samples showing presumptive positives are washed as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
