What it is, How it works

Hair serves as a long-term archive of alcohol and other substances, capturing biomarkers within the fibers of the developing hair shaft. When taken from near the scalp, hair can offer up to a 3-month detection period for alcohol and drugs. Hair is straightforward to collect, hard to tamper with, and easy to transport.

A 1.5-inch segment comprising about 200 strands of hair (about the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample using a jeweler’s precision scale. In the absence of scalp hair, an equivalent quantity of body hair may be taken. When we reference head hair, we specifically mean scalp hair. Body hair denotes all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab’s system. This process includes confirming the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available through an electronic chain of custody.

Screening involves a preliminary quick test for drug presence. While Screening is a cost-effective method to exclude drug use in most samples, a confirmed positive screen is necessary for it to be admissible in legal proceedings. Any samples that test presumptively positive in Screening require a secondary confirmation.

If a sample tests presumptively positive during the Screening phase, extra hair is extracted from the original sample and prepped for Extraction. In this phase, substances are extracted from hair at concentrations much lower than other techniques (urine or oral fluid), making hair drug testing the most challenging method to execute.

Confirmation of any positive Screening result is performed through techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as needed. The complete lab procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair specification and the ISO/IEC 17025 accreditation standards.
