What it is, How it works

Hair testing is increasingly viewed as a powerful means for detecting drug and alcohol use. It traps biomarkers within the fibers of the growing hair, offering a long-term history of substances consumed. When collected close to the scalp, hair can reveal a detection window of up to about 3 months for alcohol and other drugs. Its collection is straightforward, it's relatively tough to adulterate, and it's easy to ship.

A 1.5-inch sample consisting of approximately 200 strands of hair (roughly the size of a #2 pencil) near the scalp provides 100mg, the optimal sample size for screening and conformation processes. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the hair is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, a comparable quantity of body hair can be used. When talking about head hair, we specifically mean scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The drug test in a lab involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

The first step, Accessioning, incorporates the initial processing of a sample into the lab's system. This ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any additional data entry not covered by an electronic chain of custody system.

Screening offers an initial swift evaluation for drugs of abuse. While Screening is a budget-friendly method to rule out drug use in most samples, a positive result in screening mandates confirmation for court admissibility. Samples suggestively positive in Screening necessitate a secondary confirmation.

If a sample is presumably positive during Screening, additional hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than with other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Confirmation of any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. Suspected positive samples are washed before confirmation if necessary. The whole lab process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
