Hair collection is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch portion from around 200 strands closest to the scalp will yield approximately 100mg of hair, ideal for screening and confirmation. For tests such as EtG, add-ons, or more than 10 panels, a 150mg sample is preferred. We advise measuring the specimen with a jeweler's scale. If scalp hair cannot be obtained, an equivalent amount of body hair is suitable. By head hair, we mean only the scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test comprises Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s framework. This includes ensuring the sample’s seal and shipment were intact, issuing a random LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry not managed by an electronic chain of custody system.

Screening entails a quick preliminary check for drugs of abuse. While cost-effective in ruling out drug usage for most samples, a positive screen must be verified for court admissibility. Any samples testing positive in Screening must undergo secondary confirmation.

If Screening tests result in a presumptive positive, additional hair is extracted from the initial sample for Extraction. In Extraction, drugs are removed from the hair at lower concentrations than with other methods (e.g., urine or oral fluid), making hair drug testing the most challenging to conduct.

Confirmation of positive Screening results occurs using GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are thoroughly washed before confirmation if necessary. The laboratory process from Accessioning to Confirmation is reviewed against both the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
