When gathered near the scalp, it can reveal a drug and alcohol usage period of up to approximately 90 days. Hair is easy to gather, rather challenging to tamper with, and convenient to transport.

A sample of 1.5 inches consisting of around 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg, which is the perfect sample for both screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, 150mg is advised. It's advisable to weigh the hair sample using a jeweler’s scale. In instances where scalp hair isn’t available, an equivalent amount of body hair is acceptable. When discussing head hair, we solely mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processes a drug test through Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of processing a sample into a lab's system. This phase includes verification of the sample's proper sealing and shipment, generation of a random LAN (Laboratory Accessioning Number), and completion of any lacking data entry not covered by an electronic chain of custody system.

Should a sample yield a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction phase. In this stage, drug extraction from hair happens at significantly lower concentrations than in other methodology such as urine or oral fluid, making hair drug testing the most methodologically challenging.

Confirmation of positive Screening results is done through GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive samples undergo washing before the confirmation as necessary. The end-to-end lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.
