A 1.5-inch lock of around 200 hairs (approximately the diameter of a #2 pencil) from near the scalp provides 100mg, which is the perfect amount for screening and verification. For EtG, additional analyses, and/or tests exceeding 10 panels, a 150mg specimen is advised. It's best to weigh the specimen using a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair can be used. By head hair, we mean scalp hair specifically. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four core stages in processing a laboratory drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the entry process of the sample into a lab's system. This involves checking the sample's seal and shipping status, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry not covered by an electronic chain of custody.

Screening consists of a preliminary swift test for drugs. Screening is cost-effective for excluding drug use in most samples; however, positive results need confirmation for legal admissibility. Any samples that test presumptively positive in Screening require further confirmation.

If the Screening process indicates a presumptive positive, more hair is extracted from the original specimen for Extraction preparation. Here, drugs are withdrawn from hair at a notably lower concentration than other methods (e.g., urine or oral fluid), highlighting hair drug testing as the most challenging method.

Confirmation for any positive screening result uses GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples testing presumptively positive undergo washing before confirmation as required. The whole laboratory procedure from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
