A sample measuring 1.5 inches in length consisting of about 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, the preferred sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We propose using a jeweler’s scale to weigh the sample. If scalp hair isn’t available, a similar quantity of body hair may be used. Referring to head hair specifies only scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory system. This step includes verifying that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening provides an initial rapid check for substances of abuse. Despite being a cost-effective option to initially rule out drug use in the majority of samples, a positive screen needs confirmation for legal admissibility. Any samples showing presumptive positivity at this stage require further confirmation.

If a sample shows presumed positivity in the Screening phase, additional hair is extracted from the initial sample for Extraction. At this stage, substances are pulled from hair at a much lower concentration compared to other technologies (such as urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any presumptive positive results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are washed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
