Gathering a 1.5-inch segment consisting of around 200 hair strands (approximately the diameter of a #2 pencil) proximal to the scalp yields a 100mg ideal sample for testing and verification. For EtG, additional tests, or panels exceeding 10, a 150mg specimen is preferred. Utilizing a jeweler’s scale for weighing the specimen is advisable. In cases where scalp hair is inaccessible, an equivalent quantity of body hair can be used. 'Head hair' pertains specifically to scalp hair. 'Body hair' encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory phases for generating a drug test outcome consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial intake of a sample into a lab's system, verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional necessary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid evaluation for substance abuse. While cost-efficient for eliminating drug consumption in most samples, a positive result in screening mandates confirmation for court admissibility. Presumptively positive samples in Screening necessitate secondary confirmation.

When a sample yields a presumptive positive at the Screening stage, additional hair from the original sample is required and prepared for Extraction. This phase involves removing drugs from hair at a significantly lower concentration compared to methods like urine or oral fluid, which underscores hair drug testing as a challenging procedure.

GC/MS, GC/MS/MS, or LC/MS/MS are used to confirm any positive findings from screening. Prior to confirmation, all presumptively positive samples undergo washing as needed. The comprehensive lab process from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
