When taken from near the scalp, hair can reveal a roughly three-month period of alcohol and other drugs usage. Hair collection is straightforward, relatively tamper-resistant, and easy to transport.

A 1.5-inch segment consisting of about 200 strands (roughly akin to a #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-ons, or analyses beyond 10 panels, 150mg is advisable. Using a jeweler’s scale to weigh the sample is suggested. If scalp hair isn't an option, an equivalent amount of body hair may be gathered. "Head hair" pertains solely to scalp hair, whereas "body hair" includes all others (facial, axillary, etc.).

Process Overview

The four crucial phases in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering a sample into the lab's system. This step verifies that the sample was adequately sealed and sent, a unique LAN (Laboratory Accessioning Number) is allocated, and any supplementary data entry not covered by an electronic chain of custody is completed.

Screening consists of a preliminary rapid examination for drugs of abuse. Screening is economical for eliminating drug usage in most samples, though a positive result requires confirmation for legal use. Samples showing presumptive positive results in Screening necessitate additional confirmation.

If a sample appears positive during the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. Here, drugs are extracted from hair at far lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug testing a challenging methodology.

Confirmation for positive screenings is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed prior to confirmation as necessary. The complete process from Accessioning to Confirmation is evaluated in compliance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
