What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair acts as a long-term recorder of alcohol and drug intake by embedding biomarkers in the strands of hair as they grow. When taken from near the scalp, hair can display up to roughly a 3-month timeline for detecting alcohol and drugs. Collecting hair is uncomplicated, challenging to tamper with, and convenient for transport.

A 1.5-inch segment comprising around 200 hair strands (comparable to a #2 pencil's girth) nearest to the scalp yields about 100mg, which is the perfect quantity for testing and authentication. For EtG, supplementary, and/or assessments exceeding 10 panels, 150mg of hair is advised. Measuring the hair on a jeweler's scale is suggested. Should scalp hair not be an option, the same amount of body hair can be used. By head hair, we mean scalp hair alone, while body hair covers other types (facial, axillary, etc.).

Process Overview

The lab's processing of drug test outcomes comprises four essential stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is processed initially into a lab's system, ensuring proper sealing and shipment, issuing a random LAN (Laboratory Accessioning Number), and finalizing any extra data tasks absent from an electronic custody system.

Screening provides a speedy preliminary check for substances of abuse. Although Screening efficiently rules out drug use for many samples at a lower cost, any positive findings must be verified to serve in legal proceedings. Samples showing presumptive positive results in Screening necessitate a subsequent verification.

For samples with a presumptive positive in Screening, more hair is gathered from the original sample for Extraction. In Extraction, drugs are isolated from hair but at significantly lower levels than in other techniques (e.g., urine or saliva), highlighting the complexity of hair drug screening compared to others.

Any positive findings in Screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to Confirmation, all presumptively positive samples are cleaned as required. Throughout the entire process, from Accessioning to Confirmation, the lab follows both CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 compliance standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
