In the absence of scalp hair, an equivalent quantity of body hair can be used. The term head hair solely refers to scalp hair, while body hair encompasses all other types such as facial and underarm hair.

Process Overview

The laboratory drug testing process consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves recording the sample in the lab’s system initially. This step includes confirming the sample's integrity and proper shipping, assigning a unique LAN (Laboratory Accessioning Number), and performing any necessary manual data entry not covered by an electronic chain of custody.

Screening involves an initial assessment for drug presence. While cost-effective and efficient for most samples, a positive screen requires confirmation for legal validity. Samples that show a presumptive positive result in the Screening require a follow-up confirmation.

When a sample tests presumptively positive at the Screening stage, more hair is extracted from the original collection for further analysis. Here, hair is subjected to Extraction, where substances are isolated at lower concentrations than other methods (such as urine or saliva testing), making it the most challenging method to implement.

To validate any positive results from Screening, methods like GC/MS, GC/MS/MS, or LC/MS/MS are utilized for Confirmation. Samples that are presumptive positive undergo necessary washing before confirmation. The entire process, from Accessioning to Confirmation, is assessed in compliance with CAP (College of American Pathologists) Hair accreditation and ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
