What it is, How it works

Hair analysis has gained recognition as a potent instrument for identifying substance and alcohol use. Hair records a prolonged history of alcohol and drugs through the entrapment of biomarkers within the fibers of the developing hair strand. When trimmed close to the scalp, it can reveal up to an estimated 3-month detection period for substances and alcohol. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (analogous in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample for testing and verification. For EtG, additional tests, and/or tests exceeding 10 panels, it is advised to collect 150mg of the specimen. A jeweler’s scale is recommended for specimen weighing. If scalp hair is not present, a comparable amount of body hair may be gathered. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample into a lab’s system. This step verifies proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not included via an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient means for ruling out drug use in most samples, a positive screen must be confirmed for court admissibility. Any samples deemed presumptively positive in Screening necessitate secondary confirmation.

Presumptive positive samples during Screening have additional hair extracted from the original specimen for the Extraction phase. Here, drugs are extracted from hair at concentrations much lower compared to other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging method to carry out.

Positive screening results are confirmed using GC/MS, GC/MS/MS or LC/MS/MS. Presumptively positive samples are washed before confirmation if necessary. The laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
