What it is, How it works

Hair sampling is now recognized as an effective method for identifying drug and alcohol consumption. Hair strands serve as a long-term record of alcohol and drug usage by capturing biomarkers in the fibers during growth. By collecting hair near the scalp, it is possible to detect alcohol and drugs for up to approximately three months. Collecting hair is straightforward, hard to tamper with, and convenient to send.

Obtaining a 1.5-inch segment with roughly 200 hair strands (comparable to the thickness of a #2 pencil) from the scalp yields about 100mg, which is the optimal quantity for both screening and verification. For EtG and more extensive tests beyond 10 panels, a 150mg sample is advisable. We suggest using a jeweler’s scale for precise specimen measurement. If scalp hair isn't available, an equivalent quantity of body hair can be used. When we talk about head hair, it specifically means scalp hair. Body hair includes all other types (like facial or axillary hair).

Procedure Summary

The main phases in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of the sample into the lab’s database. This step includes confirming that the sample was securely sealed and dispatched, allotting a random LAN (Laboratory Accessioning Number), and completing any extra data input not available through an electronic chain of custody.

Screening involves a preliminary rapid examination for substances of abuse. Although Screening is a cost-saving approach for negating drug use in most samples, a positive Screening result must be substantiated for legal admissibility. Samples indicating presumptive positivity in Screening require further confirmation.

In instances of presumptive positivity during Screening, additional hair is extracted from the original sample and readied for Extraction. During this step, substances are extracted from hair at concentrations much lower than other methods (such as urine or oral fluid), making hair drug screening the most challenging technique to execute.

Any positive screening result undergoes Confirmation via GC/MS, GC/MS/MS or LC/MS/MS. All samples indicating presumptive positivity are cleansed prior to confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is conducted under both CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
