When collected near the scalp, hair can reveal drug and alcohol use for roughly up to 3 months. Hair collection is straightforward, relatively resistant to tampering, and simple to transport.

A 1.5-inch segment consisting of about 200 hair strands (roughly comparable to a #2 pencil's width) nearest to the scalp yields 100mg of hair, which is the optimal sample mass for both screening and confirmation. For tests like EtG, additional panels beyond 10 require 150mg of the sample. We advise using a jeweler’s scale to measure the sample. If no scalp hair is available, a comparable amount of body hair can be used. "Head hair" specifically implies scalp hair, while "body hair" refers to all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary stage where a sample is introduced into a laboratory’s system. This step ensures that the sample was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody system.

Screening provides an initial rapid assessment for drug use. Although Screening offers a low-cost method to rule out drug use for most samples, any positive outcome requires confirmation for legal acceptance. Presumably positive samples from Screening need further verification.

For any sample deemed presumptively positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. At this stage, substances are derived from hair in concentrations notably lower than other methods (like urine or oral fluid), thereby making hair drug tests particularly challenging to execute.

The confirmation of positive screenings is executed via GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples that are presumptive positives undergo washing before confirmation if deemed necessary. From Accessioning to Confirmation, the entire laboratory workflow is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation in line with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
