What it is, How it works

Hair testing is recognized as an efficient method for identifying drug and alcohol consumption. Hair captures a prolonged history by securing biomarkers within its growing strands. When gathered near the scalp, hair can offer up to a 90-day detection period for substances. Collecting hair is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch sample consisting of roughly 200 hair strands (about a #2 pencil in size) closest to the scalp will provide 100mg of hair, the preferred amount for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, it is recommended to collect 150mg of hair. We suggest weighing the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair can be used. Head hair specifies scalp hair, whereas body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the preliminary integration of a sample into the laboratory's system. This step checks if the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry not provided by an electronic chain of custody system.

Screening performs an initial rapid check for drugs of abuse. While it's a budget-friendly strategy to exclude drug usage in most samples, any positive findings in Screening must be verified for court acceptance. Samples that test positive prescreening require an additional confirmation.

For samples presumed positive during Screening, more hair is retrieved from the primary specimen for Extraction. At this phase, drugs are isolated from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most complex methodology.

Confirmation of any positive screen result takes place via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleansed prior to confirmation as required. The entire process from Accessioning to Confirmation is overseen under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
