Their main advantage lies in the long 90-day detection window, conducted via supervised collections, and the near impossibility of cheating. Nevertheless, for drug use detection within the last 5 days, urine tests remain the more precise option.

What it is, How it works

Hair analysis is recognized as a significant method for identifying drug and alcohol usage. Hair serves as a long-lasting record of alcohol and other substances by preserving biomarkers within the strands of hair as it grows. When gathered close to the scalp, hair can offer up to roughly a 3-month detection timeframe for alcohol and other drugs. Hair is straightforward to gather, somewhat resistant to tampering, and simple to transport.

A 1.5-inch segment of roughly 200 strands (similar in size to a #2 pencil) close to the scalp will provide 100mg of hair, which is the optimal quantity for both screening and confirmation tests. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It's advisable to measure the sample on a jeweler’s scale. If scalp hair isn't available, an equivalent quantity of body hair can be collected. When mentioning head hair, we refer exclusively to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab’s system. This involves checking that the specimen was correctly sealed and delivered, allotting a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic custody chain system.

Screening provides an initial rapid assessment for drugs of abuse. While Screening cost-effectively eliminates drug use in most samples, a positive screen needs confirmation to be legally permissible. Samples testing presumptively positive during Screening necessitate further confirmation.

In instances where a sample tests presumptively positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other approaches (ex. urine or oral fluid), making hair drug screening among the most challenging methods to perform.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
