What it is, How it works

When taken near the scalp, hair reveals up to a 3-month detection period for alcohol and other drugs. Collecting hair is straightforward, relatively tamper-resistant, and simple to transport.

A sample of 1.5 inches containing roughly 200 hair strands (approximately the width of a #2 pencil) near the scalp yields 100mg, the preferred quantity for both screening and confirmation. For EtG, add-ons, and/or assessments exceeding 10 panels, a 150mg sample is advisable. We advise the use of a jeweler’s scale to measure the specimen. If scalp hair is not accessible, an equivalent amount of body hair can be used. In terms of head hair, we specifically mean scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of drug test results includes four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial integration of a sample into a lab's system. This process ensures the sample is sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any further data not submitted via an electronic chain of custody.

Screening is a preliminary rapid evaluation for drug abuse. Although Screening is an economical approach for excluding drug usage in most samples, any positive outcome requires confirmation to be legally acceptable. Samples with a presumptively positive result in Screening necessitate a secondary validation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for Extraction. Here, drugs are retrieved from hair at a much lower concentration compared to other methods (like urine or oral fluid), which makes hair drug testing the most complex to conduct.

Confirmation of positive Screening outcomes is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed before confirmation if necessary. The whole lab process, from Accessioning to Confirmation, is supervised under both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 standards adherence.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
