What it is, How it works

Hair testing is now widely recognized as a robust method for detecting drug and alcohol consumption. Hair captures long-term evidence of alcohol and various drugs by embedding biomarkers within the fibers of the hair strand as it grows. When taken from near the scalp, hair samples can offer up to a 3-month detection span for alcohol and drugs. The collection process is straightforward, tampering is somewhat challenging, and it’s easy to ship.

A 1.5-inch segment of roughly 200 hairs (resembling a #2 pencil's thickness) nearest to the scalp yields 100mg of hair, which is the best sample size for both screening and confirmation. For EtG, add-ons, or tests above 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not obtainable, the same quantity of body hair can be collected. By head hair, we mean only scalp hair. Body hair refers to any other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase of processing a sample within a lab’s framework. This step involves checking that the sample was sealed and mailed correctly, assigning a random LAN (Laboratory Accessioning Number), and recording any additional information not captured by an electronic chain of custody system.

Screening is a quick initial assessment for drugs of abuse. Although Screening is a cost-effective method for ruling out drug use in most samples, a positive result must be confirmed to be valid in court. Any samples that test presumptively positive in Screening require additional verification.

If a sample shows a presumptive positive during Screening, more hair is extracted from the original specimen and readied for Extraction. Here, drugs are detached from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most intricate method to execute.

Confirmation of any positive screening outcome is done through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair classification and meets the accreditation of ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
