A sample size of 1.5 inches, consisting of about 200 hair strands (similar to a #2 pencil thickness) cut nearest to the scalp, provides 100mg of hair, suitable for both initial screening and further verification. For EtG, additional tests, and/or tests exceeding 10 panels, it is suggested to use 150mg of the sample. We suggest employing a jeweler’s scale for weighing the sample. If scalp hair isn’t available, a comparable quantity of body hair may be used. The term head hair specifically refers to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing for drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample within a laboratory’s system. This step involves checking that the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient means to exclude drug use in most samples, a positive result requires confirmation to be valid in legal settings. Samples that initially show positive in Screening need a subsequent verification.

Should a sample display a positive indication in the Screening phase, additional hair is taken from the original sample for Preparation for Extraction. During Extraction, substances are retrieved from hair at a notably lower concentration compared to some other methods (such as urine or oral fluid), which is why hair drug analysis is the most challenging methodologically.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation as needed. The comprehensive laboratory workflow from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 regulations.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
