A sample measuring 1.5 inches consisting of roughly 200 hair strands (comparable to the dimensions of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for screening and verification. For EtG, supplementary analyses, and/or tests exceeding 10 panels, a 150mg specimen is advised. It is recommended to weigh the specimen with a jeweler’s scale. If scalp hair is inaccessible, an equivalent quantity of body hair may be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample within a laboratory’s framework. This involves verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody system.

Screening involves an initial swift examination for drugs of abuse. Although Screening serves as a cost-efficient approach to rule out drug consumption in most samples, a positive screen necessitates confirmation to be permissible in court. Samples that are preliminarily positive in Screening require a secondary verification.

Upon a presumptive positive indication in Screening, additional hair from the initial specimen is prepared for Extraction. This phase involves extracting drugs from hair at a significantly lower concentration than alternative methodologies (e.g., urine or oral fluid), which accounts for the complexity of hair drug screening.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All presumptive positive samples undergo a washing process before confirmation as required. The comprehensive laboratory workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
