When obtained near the scalp, it can indicate alcohol and drug use for up to roughly three months. Hair is easy to gather, moderately resistant to tampering, and convenient for transportation.

A 1.5-inch hair sample, approximately 200 strands (similar in size to a #2 pencil) closest to the scalp, yields 100mg of hair, the optimal quantity for evaluation and verification. For EtG additions and/or analyses exceeding 10 panels, a specimen of 150mg is advisable. We suggest measuring the specimen using a jeweler’s scale. If head hair is unavailable, a comparable quantity of body hair may be used. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is processed into the laboratory’s system. Tasks include verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional information not included in an electronic chain of custody system.

Screening offers a preliminary check for drug abuse. Although cost-effective for eliminating drug presence in most samples, positive results from Screening require confirmation to be legally valid. Samples registering as presumptively positive during Screening necessitate additional verification.

If a sample tests presumptively positive in Screening, additional hair is extracted from the original specimen and prepared for Extraction. Drugs are extracted from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging to perform.

Confirmation of any positive results from Screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned prior to confirmation as necessary. Each step from Accessioning to Confirmation is reviewed under the oversight of both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
