What it is, How it works

Hair analysis is increasingly regarded as an effective method for identifying drug and alcohol usage. The hair can capture long-term evidence of alcohol and drug presence by embedding biomarkers within the hair's fibers as it grows. If taken from near the scalp, hair offers an approximate 3-month window for alcohol and drug detection. Hair is straightforward to collect, hard to tamper with, and simple to transport.

A 1.5-inch section containing roughly 200 hair strands (similar in thickness to a #2 pencil) from the scalp area can produce 100mg, which is the optimal sample size for both screening and validation tests. For EtG, additional testing, and/or analyses exceeding 10 panels, 150mg of the hair sample is advised. Measurement of the sample should ideally be carried out using a jeweler’s scale. In cases where scalp hair isn't available, a comparable quantity of body hair may be collected. When mentioning head hair, it exclusively refers to scalp hair, while body hair includes all other hair types (such as facial and axillary hair).

Process Overview

The laboratory processing of drug test outcomes comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial sample processing into the laboratory system. This process verifies that the sample is correctly sealed and dispatched, assigns it a unique LAN (Laboratory Accessioning Number), and completes any extra data entry that isn't included in an electronic chain of custody protocol.

Screening is a preliminary rapid test for substance abuse. Though it is economical for ruling out drug usage in most samples, a positive screen mandates confirmation for court admissibility. Samples that are putatively positive during Screening necessitate subsequent confirmation.

If a sample is tentatively positive at the Screening phase, a further portion of hair is extracted from the initial batch for preparation in the Extraction phase. Drugs are then recovered from the hair in much lower concentrations than in alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Positive results from the Screening phase are validated using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All potentially positive samples undergo a washing process as necessary before confirmation. The complete laboratory process, from Accessioning through Confirmation, is scrutinized. This evaluation is consistent under both the CAP (College of American Pathologists) Hair program and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
