What it is, How it works

When collected near the scalp, hair can offer an approximate detection window of up to 3 months for alcohol and drugs. It is easy to collect, resilient against tampering, and straightforward to transport.

A sample measuring 1.5 inches with around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal amount for both screening and confirmation purposes. For EtG, additional panels, or tests exceeding 10 panels, a 150mg sample is advised. We suggest measuring the sample using a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair may be used. When we mention head hair, we specifically mean scalp hair, while body hair refers to any other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of the sample into a laboratory’s system. This step includes ensuring the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not available through an electronic chain of custody system.

Screening provides a preliminary assessment for drugs of abuse. While cost-effective for eliminating drug usage in most samples, a positive screening must be confirmed to be court-acceptable. Any samples showing presumptive positivity in Screening require further confirmation.

Should a sample test positive during the Screening stage, additional hair is taken from the original specimen for the Extraction process. Here, drugs are extracted from hair at considerably lower concentrations than in methods such as urine or oral fluid testing, which is why hair drug screening is renowned as one of the most complex methodologies.

Confirmation of a positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo washing before confirmation if needed. From Accessioning to Confirmation, the entire laboratory workflow is checked under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
