An ideal sample for screening and confirmation is a 1.5-inch segment containing roughly 200 hair strands (comparable to a #2 pencil), yielding 100mg of hair. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is advisable. It's suggested to use a jeweler's scale for accurate measurement. If scalp hair is not an option, a similar quantity of body hair can be collected. "Head hair" pertains only to scalp hair, whereas "body hair" refers to hair from other areas (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result encompasses four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of sample integration into a lab system. This phase entails confirming the proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any absent data entry not covered by an electronic chain of custody system.

Screening is a swift initial assessment for substance abuse. Although Screening efficiently excludes drug use in most samples, a positive result requires confirmation for legal admissibility. Samples deemed presumptively positive in Screening necessitate subsequent confirmation.

With a presumptively positive result in Screening, additional hair is derived from the initial sample for Extraction. In this phase, drugs are isolated from hair at concentrations significantly lower than those in other procedures (e.g., urine or oral fluid), rendering hair drug screening the most challenging to execute.

GC/MS, GC/MS/MS, or LC/MS/MS are employed for confirming any positive screening outcomes. All presumptive positive samples are cleansed before confirmation as required. Every laboratory phase from Accessioning through Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
