For EtG, supplementary tests, and/or analyses exceeding 10 panels, a 150mg sample is preferable. It's recommended to weigh the sample using a precision jeweler's scale. If scalp hair cannot be obtained, an equivalent quantity of body hair may be gathered. The term head hair specifically denotes scalp hair only. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a specimen into the laboratory's system, ensuring the sample was correctly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and inputting any additional data absent from an electronic chain of custody record.

Screening involves a preliminary examination for substances of abuse. Any samples with a presumptive positive result in Screening must undergo a subsequent confirmation.

Should a specimen test as presumptively positive during Screening, further hair is extracted from the initial sample and prepped for Extraction. Here, drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any affirmative screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS methodologies. All presumptive positives are cleansed before confirmation as required. The complete laboratory evaluation from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair standards and is certified under ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
